As a Volunteer Transplant Education Support Navigator, you will:
Help deliver educational material to patients and families, including brochures, videos and web-based materials, over a series of visits with patients
Offer support and encouragement to people living with chronic kidney disease, helping them access services and connecting them with staff who can best meet their needs, which can be immensely beneficial to people facing the burden of illness
Contribute to quality improvement as part of an interdisciplinary team supporting patients, collecting data tracking the impact of this education, and transforming patients’ experience of receiving transplant education
You will need to be able to commit to being a Navigator for at least 1 full year. To ensure a high-quality education experience for patients, you will need to be available 10 hours per week for 2-3 shifts of 4 hours each, preferably on a consistent day(s) and time each week (Monday through Saturday, between 7 am and 5 pm).
