Become a Transplant Education Support Navigator

The Enable Access to Kidney Transplant – Living Kidney Donation (ENAKT-LKD) initiative aims to enhance health education provided to Ontario patients with kidney failure, and their access to transplant. We seek motivated, eager individuals with a passion for education and health advocacy to become Volunteer Transplant Education Support Navigators to support kidney transplant education for patients with advanced kidney disease at one of these 8 hospitals:

  • St. Joseph’s Healthcare Hamilton
  • Trillium Health Partners Etobicoke
  • St. Joseph’s Health Centre Toronto
  • Humber River Regional Hospital
  • St. Michael’s Hospital
  • Sunnybrook Health Sciences Centre
  • Mackenzie Health Richmond Hill
  • Peterborough Regional Health Centre

As a Volunteer Transplant Education Support Navigator, you will:

Help deliver educational material to patients and families including brochures, videos and web-based materials, over a series of visits with patients

Offer support and encouragement to people living with chronic kidney disease, helping them access services and connecting them with staff who can best meet their needs, which can be immensely beneficial to people facing the burden of illness

Contribute to quality improvement as part of an interdisciplinary team supporting patients, collecting data tracking the impact of this education, and transforming patients’ experience of receiving transplant education

To be a successful Navigator, you must be:

  • At least 20 years old and eager to learn about transplant
  • An empathetic, responsible person with strong interpersonal skills
  • Able to speak English well (fluency in other languages an asset)
  • Able to fulfill volunteer screening requirements at the local hospital site
  • Able to commit to being a Navigator for at least 1 full year
  • Available 10 hours per week for 2–3 shifts 4 hours each, preferably at the same day/time (Monday-Saturday, between 7 am–5 pm)

If approved, you will complete the local hospital’s volunteer screening and orientation process, undergo a police record check, and be provided with Navigator training on how to deliver kidney transplant navigation in a sensitive, appropriate manner.
